Social health and care pilot programmes making a difference to people’s lives and care

Over recent years, Bedfordshire, Luton and Milton Keynes (BLMK) Integrated Care System’s Digitising Social Care (DiSC) programme has introduced a number of innovative pilot projects with new ways to support our residents.

CBC social care Several of these projects are already having a significant impact on individuals, their caregivers, loved ones, and in our care homes. Our aim is to help prevent the problems that lead to serious health issues and stays in hospital.
